Does insurance cover IVF, IUI or egg freezing? Check by state
Whether a plan covers fertility treatment depends on the state, the type of plan and the employer. We do not summarize statutes ourselves. We point you to ASRM's page for your state and to the questions worth asking your insurer.

Questions to ask your insurer or HR
- Is my plan fully insured or self-insured (self-funded)? Does the state's fertility law apply to it?
- Which fertility services are covered: diagnosis, monitoring, IUI, IVF, medications, egg freezing, storage?
- Are there limits (number of cycles, lifetime dollar caps, age or diagnosis requirements)?
- Do I need prior authorization, and must I use in-network clinics or a specific pharmacy?
- Can you confirm the answers in writing, with the plan document section?
Common questions
Does insurance cover IVF?
It depends on your state's law and your specific plan. ASRM maintains a state-by-state page on infertility insurance laws, and notes that the federal ERISA law exempts companies that self-insure from state regulation, so an employer's self-insured plan may not follow your state's rules. Confirm with your plan administrator in writing.
Does insurance cover egg freezing?
ASRM's patient fact sheet says "Some but not all insurance plans cover elective egg freezing". Check your plan's benefits and conditions, and ask separately about medication and storage.
Where can I look up my state's infertility insurance law?
ASRM's ReproductiveFacts.org hosts a page for each state and territory. Use the state links on this page; each one opens ASRM's page for that state. For questions about your state, ASRM points readers to the state's Insurance Commissioner's office.
